CPAP Club AirSense11 Cashback Redemption Claim Form
Terms and Conditions
1. This Cashback Offer (“Cashback Offer”) is provided by CPAP Club Pty Ltd, ABN 99 622 428 942.
2. This Cashback Offer begins, 20 February 2024 and ends 11:59pm 02 April 2024 (“Offer Period”).
3. Any payment made in accordance with this Cashback Offer will be made in the form of a prepaid $100 GO Visa card, provided by Prezzee, delivered to your nominated email address or mobile phone number via purchaser redemption. You are accepting the Prezzee GO Visa Card terms and conditions by accepting the GO Visa Card from Prezzee.
4. Any person who participates in the Cashback Offer acknowledges it is made subject to these terms and conditions and will be deemed to have accepted these terms and conditions.
Eligibility for the Cashback Offer
5. A purchaser will be eligible for the Cashback Offer if the purchase satisfies all of the following conditions:
a. Purchase of any model of the ResMed AirSense11 Device, ResMed Code 39107 (Elite) or 39108 (AutoSet)
b. Purchased through cpapclub.com.au website during the offer period
c. Redemption claim submitted via the online form at https://www.cpapclub.com.au/cashback-claim by 03/05/2024 (“Redemption Claim Close Date”)
Exclusions & Limitations
6. The cashback offer cannot be combined with any other promotions or discounts. It is not valid for purchases made outside the Offer Period or for products other than the Eligible Products. Purchases made through our channel partners, such as marketplaces and other associated businesses of CPAP Club and InterMed Australia are not eligible. Not eligible if device has been returned under the ResMed 30-Day Guarantee or returned and refunded under any other agreement.
7. Only one cashback offer can be claimed per eligible product purchased.
Verification
8. CPAP Club reserves the right to verify the eligibility of participants and the validity of the redemption claims. Any fraudulent or invalid claims may result in the forfeiture of the cashback offer.
Cashback Offer Delivery
9. All eligible claims submitted by the Redemption Cutoff date will have their Cashback Offer ($100 Visa GO Card by Prezzee) delivered to the nominated email address or mobile phone number between 06/05/2024 – 10/05/2024. CPAP Club will not be held responsible for incorrect email address details provided by purchasers in the redemption claim form.
Changes to this Cashback Offer
10. CPAP Club reserves the right to, at any time, vary these terms and conditions provided that CPAP Club will not make any variation which would materially impact on the circumstances in which the Cashback Offer will be paid or on how it will be paid, during the Offer Period.
